I have taught myself to drive with my heel under the brake pedal, and I turn my foot out to reach the tiny gas pedal. That way, my foot is always oriented with bias toward the brake pedal.

That's what I've been doing for years but I've noticed that my foot has been getting lazy as I get older. I tend to only catch the right edge of the brake with the center of my foot. It's hard to undo 35 years of training but I'm getting there. I'm thinking that my lazy foot is partly caused by not driving a stick shift anymore (not in the past 6 years).
